Vision Benefit Overview

You may use a provider that is a member of the VSP network, or you may use the provider of your choice; however, your co-payment rate may be affected.

The Trust has five different vision benefit plans with VSP. If you are unsure of what plan you have, you can call VSP at (800) 877-7195 or log onto VSP.com to find out. Each plan has rates for VSP providers and non-VSP providers. Your collective bargaining agreement or special agreement determines which Plan you are in. Click the link below to see an overview of what is covered for each plan.

Using VSP Providers

To select a vision care provider that is a member of the VSP network, go to VSP’s website at VSP.com. To enter the site, you will need to enter the social security number and last name of the participant (employee) in the member sign-on.

Once you have selected your provider and scheduled an appointment, the doctor’s office will call VSP and confirm your coverage. If you are not listed as eligible, have the doctor contact the Administrative Office. 

When your eye exam is completed, you will be responsible for your co-payment and any cosmetic options or other services not covered by the Plan. The doctor’s office will file the claim with VSP for the covered services.

Using Non-VSP Providers

If you have or are planning to use a vision service provider that is not a member of the VSP network, you are responsible for paying the non-member bill in full.

File an Out-Of-Pocket Claim

To be reimbursed for out-of-network vision services, you must complete a Request for Reimbursement on the VSP website. Log into your account, visit the Benefits & Claims section of the site. Click the button to start your Out-Of-Pocket claim. The website will walk you through the online form. When you are finished, print out the form for you and/or each dependent and mail it, along with the itemized bill to:

Alternately, you may submit your printed form(s) from VSP and receipt(s) to the Administrative Office and they will forward it to VSP for you.

Please note: All claims must be filed within six (6) months of the date services were completed.
