Dental Benefit Overview

Most types of dental services are covered.  Dental services are divided into three parts:

  • Part I – Preventative Dentistry
  • Part II – Restorative Dentistry
  • Part III – Major Dentistry

See the Summary of Benefits in the Health & Welfare Summary Plan Description (H&W-SPD) for a list of covered services. Co-payment rates will vary depending which Part the service is categorized as, and which Plan you are enrolled. Your collective bargaining agreement or special agreement determines which Plan you are in.

Finding a Dentist

There is no preferred provider for dental benefits. You may use any dental provider without an out-of-network penalty. If you need help finding a dentist, check out this article from WebMD with tips from the American Dental Association.

Filing a Claim

If your dentist does not bill insurance, you will need to submit your receipts along with a Medical Dental Reimbursement Request Form that includes:

  • Date of service
  • Dental codes
  • Provider’s name and Tax ID number
  • Participant’s employee ID number
  • Patient’s name and Date of Birth
  • Notation that payment has been received or a receipt

Note: To ensure prompt payment, make sure the Annual Medical / Dental Update Form on file with the Admin Office is up to date for the person who received the care. A reimbursement check will be mailed directly to you when the claim is processed.
