It is necessary for the Board of Trustees to verify that our retirees and beneficiaries are receiving retirement benefits to which they are entitled. Each Retiree Signature Certification form must be signed and notarized by a Notary Public or an authorized Plan Representative, and returned to the Administrative Office by June 15, 2018.

Failure to return this form by June 15, 2018, will result in the suspension of your monthly retirement benefit effective July 1, 2018.
